Fun question. The Fourth Doctor is a reference to Dr. Who which was one of my favorite science fiction shows when I was a kid. Dr. Who is a Timelord who does not die, but regenerates into another body every so often. When I watched as a kid it was the fourth generation.

The Doctor is simply and always called the Doctor. The show still runs today. The thirteenth Doctor I believe.
